Inspector Notes
An unannounced, on-site renewal inspection was initiated on 9/8/2025 and completed on 9/8/2025. The on-site inspection began at 10:10AM and ended at 12:30PM. The inspector reviewed compliance in the areas listed above. There were 45 children present and 9 staff. The inspector reviewed 3 new children?s records and staff records on-site.
This inspection included document review, a tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. For questions regarding this inspection, please contact stephanie.salvatori@doe.virginia.gov
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #1, who had been employed for 1 week, did not have a completed national criminal background check.
Plan of Correction: We're having trouble with OBI. We will get everything set up today and send the results.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one state for Staff #1, who has been employed for one week and was working with children at the time of the inspection.
Plan of Correction: We will get everything set up and send the results as soon as they are received.
Standard 8VAC20-770-60-C-2
For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service. Caregiver #1 did not have central registry search results during the inspection and has been employed for 1 week.

Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The screening for Staff #1 was not available for review.
Plan of Correction: The TB test results will be sent into the Licensing Inspector.
