Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 17, 2023 from approximately 11:40 AM to 1:20 PM. There were 23 children in attendance and a total of six staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-600-D-1
Based on review of five staff records and interview, the vendor did not ensure all staff who work directly with children have within 90 days of the date of
employment, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training must include an in-person
competency demonstration.
Evidence:
1. The record for Staff #2 (Date of Employment 12/19/2022) did not contain documentation of CPR certification.
2. A member of management stated Staff #2 had not completed the training.
Plan of Correction: We will have the staff member sign up and take the required training.
Standard 8VAC20-790-600-D-2
Based on review of five staff records and interview, the vendor did not ensure all staff who work directly with children have within 90 days of the date of employment, current certification in first aid appropriate to the ages of children in care.
Evidence:
1. The record for Staff #2 (Date of Employment 12/19/2022) did not contain documentation of current first aid certification.
2. A member of management stated Staff #2 had not completed the training.
Plan of Correction: We will have the staff member sign up and take the required training.
Standard 8VAC20-790-670-C-1
Based on observation, the vendor did not maintain the required staff-to-child ratio of one staff member to every four children ages zero to 16 months.
Evidence:
At approximately 12:05 pm, Staff #1 and Staff #2 were observed supervising six children (five months to 13 months of age) in the classroom. Staff #2 was observed exiting the classroom, going into the adjacent room multiple times.
Plan of Correction: The children were asleep and we thought we could double ratio during nap time. The staff member stepped into the next room with the half door open and was able to see into the classroom at all times. It was reviewed with the staff members while the inspector was onsite, that the infant staff to child ratio must be maintained at all times.
Standard 8VAC20-790-820-C
Based on observation, the vendor did not to ensure bottles were not used while the child is in his/her designated sleeping location.
Evidence:
Child #1 (15 months of age) was observed lying in the crib, drinking a bottle/cup.
Plan of Correction: While the licensing inspector was onsite, it was reviewed with all staff that no bottles, drinks or food is ever allowed in the cribs.
