Inspector Notes
The licensing inspector initiated a subsidy complaint inspection on April 18, 2024, and completed on Monday, May 20, 2024, in response to a complaint received by the department on April 11, 2024, for allegations regarding forbidden action and behavioral guidance. On Thursday, April 18, 2024, the inspector was on site from 11:00am until approximately 12:40pm. Interviews were conducted, observations were made at the center, and documentation was reviewed.
Based on the information gathered during the investigation, the preponderance of evidence supports the allegation, therefore the complaint is determined to be valid. Violations were documented on the violation notice issued to the program.
Please complete the 'plan of correction' and `date to be corrected' for each violation cited on the violation notice and return it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Violations
1Evidence: During interview, Staff #1 reported that on 4/10/24, she pinched Child #1 (4-year-old), after he said to Staff #2 ?you better not pinch me.? Staff #1 reported she did not like the child?s tone, so she decided to pinch him. The staff and management acknowledged this was not constructive in nature or age and stage appropriate.
After this violation happened, Staff #1 took a training on May 4, 2024, called "Take Care of You and Taken On Challenging Behaviors." The training gave positive methods to appropriately correct undesired behavior in children according to the age of the child. This training also emphasized ways to relieve caregiver stress that Staff #1 has resolved to put into practice.
The outgoing Director will line up additional training classes and will monitor that steps for correction are put into practice. Sessions will be scheduled regularly to review healthy interaction with children and to review Center policies."
