Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A monitoring inspection was initiated on 9/14/2021 and concluded on 9/14/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 109 children present, ranging in ages from 4 months to 4 years, with 32 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #1's (FDA: 6/14/2021) record does not contain documentation to verify that the child has received the immunizations required by the State Board of Health prior to their first day of attendance.
Plan of Correction: Sent letter to parents requesting immunization record on 9/14/2021.

Based on record review, the center did not obtain documentation of a physical examination by or under the direction of a physician, within 30 days of attendance.

Evidence:
Child #1's record did not contain documentation of a physical examination. The child's first day of attendance was documented as 6/14/2021.
