Inspector Notes
An unannounced renewal inspection was conducted today from 10:30am-12:35pm. There were 125 children (infants-5yrs.) directly supervised by 32 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, show and tell, singing and counting activities, listening to stories and playing outside. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Two holes, approximately 3 inches in diameter, were observed at the bottom of the hallway wall, adjacent to the Monkey's classroom.
Evidence:
In the Monkey's classroom, two cans of Lysol disinfectant spray were observed unlocked on a high shelf next to the bathrooms.
Evidence:
An evacuation drill was not practiced for the month of November 2022.
