During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/9/2024, as a part of the licensure period. The on-site inspection began at 12:54pm and ended at 2:50pm. The inspector reviewed compliance in the areas listed above. There were 117children present and 35 staff. The inspector reviewed 5 children?s records and 5 staff records onsite.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/18/24. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Peeling paint was observed near the cot storage area, and would flake away when touched and land on the cots, in the Monkeys classroom.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Bleach water and soap water were on the counter and unlocked during the children's naptime in the Toucans classroom. Lysol and Febreez were on a shelf accessible to children in the Toucans classroom. Hand sanitizer was on a low shelf accessible to children in the Monkeys classroom.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

There were several bottles in the Butterflies refrigerator, and none of them were dated.

The following staff records shall be kept for each staff person: 2. For staff hired after March 1, 1996, documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
