Inspector Notes

An unannounced monitoring inspection was initiated and completed on 9/26/2025 in response to a self-report received by the licensing office on 9/14/2025 relating to supervision. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included: document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/5/2025. A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 9/14/2025, staff 1 left their classroom through a side door, entering classroom 2, and going outside to the playground to address teachers on the playground. child 1 walked into classroom 2, unseen. Staff 1 entered classroom2, and then went back into classroom 1, leaving child 1 alone in classroom 2. Child 1 was alone and unsupervised in classroom 2 for approximately 7 minutes. Staff 2 and 3 realized that child 1 was not with their group, and looked in classroom 2, finding the child playing quietly at a table.
Plan of Correction: Side door is to remain closed at all times. The appropriate people were spoken to and all is well. Count children multiple times throughout the day.
