Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 01/22/2025, as a part of the licensure period. The on-site inspection began at 10a.m. and ended at 11:30a.m. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 33. The inspector reviewed 6 children?s records and 3 caregivers and 2 household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child.

This information was not available for Child #6 who was present during the inspection and began care on 01/06/2025.
