Inspector Notes

An unannounced renewal inspection was conducted from 4:30 - 5:00 pm with staff. There were 26 children, ranging from five years to ten years of age, present with four staff supervising. Children were observed eating snack and participating in a variety of age-appropriate centers/activities. Snack served today: pretzels and oranges. Four onsite staff records and two onsite child records were reviewed at the time of the inspection along with three emergency medications, the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, and the required posted information. Four staff records were reviewed off site at the Massad YMCA on 11/17/2022. Based on review of four staff records, the center did not have documentation of a negative tuberculosis (TB) test/ screening for staff completed within the last 30 calendar days of the date of employment. Evidence: the record for staff #4 (date of hire: 5/25/2022) had a TB test/screening dated 9/11/2021.
Plan of Correction: Going forward, will ensure that documentation is obtained within the required time frames.
