Inspector Notes

An unannounced renewal inspection was conducted from 3:30 - 4:00 pm with staff. There were 30 children, ranging from six years to 11 years of age, present with three staff supervising. Children were observed working on an arts and crafts project, having free play and completing homework. Two children's records and three staff records were reviewed and in compliance. Staff stated there was no medication on site, however two staff have current certification in Medication Administration Training (MAT), first aid and CPR. The emergency drill log, attendance, written allergy list, first aid kit, fire and health inspections were reviewed and in compliance. Based on observation, the facility failed to maintain the required ratio of one staff for every 18 children (20 children with ratio waiver), ages five years to eight years of age. Evidence: When the LI arrived, one staff was observed alone with 27 school age children for less than two minutes. The youngest child in attendance today is six years old.
Plan of Correction: Will retrain staff to ensure ratios are met at all times.
