Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 09/13/21021 and concluded on 09/16/2021. The director was contacted by telephone to initiate the inspection. There were 59 child present and 16 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

2Evidence:
The metal stair case the school-age children use to exit to the playground and for emergencies had rust on one of the steps.
Evidence:
A staff member in the infant room was observed changing a child's diaper. The staff member did not dispose of the soiled diaper in the foot operated trash can. The soiled diaper was wrapped up and placed on the changing table while putting a clean diaper on the child.
