The licensing inspector conducted an unannounced complaint inspection on 01/06/2023 In response to a complaint that was received in the licensing regarding allegations in the area of physical plant. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. There was peeling paint in all of the classrooms and the wooden trim throughout the center was peeling and chipped.
2. The floor paneling in the gym was peeling and lifting in areas and the supply door had an area of rotting wood at the bottom hinge.
3. On the twos playground there were 2 plastic wagons that had broken pieces with jagged and sharp edges.
4. In the school -age room, there were four 1/4 inch holes in a wall.

Plan of Correction: This will be corrected in 30 days. Going forward Safety captain and management will do walk throughs to make sure all areas are maintained in a safe condition and will partner with DM to ensure follow up on maintenance.
