During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/26/24. The on-site inspection began at 9:50am and ended at 1:10pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 11 staff. The inspector reviewed six children?s records and six staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/13/24. A POC submitted after this date will not appear on the public website.

The required first aid kits shall include at a minimum all the required items.
The first aid kit for review at the center did not contain triangular bandages or tweezers, which are required.

When food is brought from home, the food contained shall be clearly dated and labeled in a way that identifies the owner.
In the two-year-old classroom, ten of 15 water bottles were not labeled to show which child uses the water bottle.

Staff records shall be kept for each staff and contain all the required information.
1.The record for Staff #1 did not contain the address of a person to be notified in an emergency.
2.The records for Staff #2, Staff 3, and Staff 4 did not contain the dates of contacts for both required references, which is required.
