When staff are supervising children, they shall always ensure their care and protection. On 12/8/25, the parent of Child #1 revised information in the child?s record to change Person #1 from authorized to pick up the child to unauthorized to pick up the child. On 12/12/25, Staff #1 allowed Person #1 to visit the child in the gym for a class activity. There were 10 other children in the gym during the visit.
Plan of Correction: On 12.12.25 A prior Authorized Grandparent entered the building and was signed into the Visitor Log at 10:26am for an Event, Management verified that the Grandparent was no longer Authorized and Grandparent was asked to leave and signed out at 10:27am. Moving forward, All Visitors will be verified prior to leaving the lobby area.
