Inspector Notes

An unannounced monitoring inspection was conducted on 11/15/23 from 6:45 a.m. to 7:30 a.m. with the Site Director. There were 10 children in care, ranging in age from five to 12-years-old, supervised by three staff. The children were observed playing table games. Four child records and three staff records were reviewed. The Center has three staff with current certification in CPR and First Aid, and three staff trained in Daily Health Observation Training. No medications were on site center has one staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. The most recent Fire Inspection on file was dated 09/20/22. Areas of non-compliance are identified in the Violation Notice. Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance.

Evidence: There were seven children present, the written record reflected there were three.
Plan of Correction: Staff have been spoken to about the importance of having every child checked in. Staff understand.
