Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 18, 2025. The on-site inspection began at 7:30am and ended at 8:45am. The inspector reviewed compliance in the areas listed above. There were 16 children present and 2 staff. The inspector reviewed four children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

Child #1's (age: 7 years) most recent medication authorization forms for an Epi-Pen and liquid Benadryl expired 8/20/2025 and the medications were on-site.
Plan of Correction: We will talk to the parent about updating the medication authorization forms.

Nonmedical emergency supplies shall be required.

The center's battery-operated radio would not turn on.
Plan of Correction: We will get a new radio.
