During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated on September 18, 2025, and completed on September 25, 2025, in response to a self-report received by the licensing office on September 9, 2025 relating to supervision. The inspector reviewed four children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On September 8, 2025, Child #4 (age: 9 years) climbed on top of a piece of outdoor play equipment and after being instructed to get down by staff two-to-three times, child #4 fell from the playground equipment, breaking their elbow.

During staff interviews, Staff A and Staff B reported that children have been climbing on top of this playground equipment for the past year.
Plan of Correction: Children a restricted from the playground until further notice.
