Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, virtual document review and interview.

A monitoring inspection was initiated and completed on 12/09/2021. The Provider was contacted by telephone and a remote inspection was completed. There were no children present with supervising staff at the time of the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programing, medication policies and procedures , special care and emergencies and nutrition. A sample of child records and staff/ household member records were reviewed.

The information gathered during the inspection determined violations with applicable standards or law. Violations were issued.

Based upon record review and interview, required TB test resubmitted every 2 years was not completed as required.
Evidence : Staff A's Tb test expired 10/1/2021 and a new screen had not been completed.
Staff B's Tb test expired 6/27/2021 and a new screen had not been completed.
Plan of Correction: We will have someone come on site or have test completed as soon as possible.
