During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was completed on 08/04/2025, as a part of the licensure period. The on-site inspection began at 8:20 am and ended at 10:10 am. The inspector reviewed compliance in the areas listed above. There were 24 children present and 7 staff. The inspector reviewed 4 children?s records and 3 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.

Staff A Tuberculosis repeat screen or testing was over due approximately 3. 5 months

The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the authorization for administration is complete and not expired.

Medication authorization for Child A was expired for less than 1 week.
Medication authorization for Child B was expired for 3 months.
Plan of Correction: Monthly checks for all long term enrollments and weekly checks for every student ( or planning to be enrolled). during the summer.
