Inspector Notes

An unannounced monitoring inspection was conducted on 05/11/2023 and concluded with an additional record review with administration staff at another location on 05/19/2023. There were 17 children, ages 4 years- 12 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, nutrition and background checks. The children were observed during physical activities, snack time, and teacher-directed activities. A total of 5 children?s records, and 4 staff records were reviewed. The inspector discussed the following with the staff: handwashing and food from home. The inspector arrived for the inspection at 3:30 pm and departed at 5:15 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to provider.

Based on record review, the center failed to obtain a criminal record check, prior to hire, from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 3 contained documentation that Staff 3 has resided outside of the state of Virginia within the preceding 5 years. The documented hire date for Staff 3 was 09/12/2022. The out of state criminal record check was dated 09/26/2022.
Plan of Correction: Staff will obtain required information prior to hire.
Standard 8VAC20-780-60-A
Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.
Evidence:
1. The record for Child 3 did not contain the following information: name address and phone number of two designated people to call in an emergency if a parent cannot be reached (there was no addresses for the second contact).
2. The record for Child 5 did not contain the following information: name address and phone number of two designated people to call in an emergency if a parent cannot be reached ( the addresses for the first contact was incomplete).
Plan of Correction: Staff will obtain information from parents.
Standard 8VAC20-780-70
Based on record review and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.
Evidence: The record for Staff 3 did not contain the following: name address and telephone number of person to be notified in an emergency and information about any health problems that my interfere with fulfilling the job responsibilities. Staff 1 confirmed this information was not available at the center, as required by the standard.
Plan of Correction: Staff will obtain information.
