Inspector Notes

An onsite code compliance inspection was conducted October 30, 2023. The licensing inspector was onsite from approximately 9:30 AM until 10:45 AM. One child was present with one staff member supervising. The child was 21 months of age. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. One child?s record and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Based on review of two staff records and interview, the center did not ensure one staff completed background checks at least every five years.
Evidence:
1. The record for Staff #2 contained a fingerprint based national criminal record check dated 07/16/2018 and a sworn statement form dated 07/16/2018.
2. Staff #2 confirmed this was accurate during the inspection.
Plan of Correction: While the inspector was onsite, an appointment was made to obtain the updated national criminal background check and an updated Sworn Statement was completed.
