Inspector Notes

A monitoring inspection was conducted on February 25, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 80 children present, ranging in ages from birth to 12 years, with 15 supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of 5 staff records, the provider did not obtain a copy of the results of a criminal history record information check from a state which an individual has living in within the past 5 years before first day of employment.
Evidence
An out of state back ground check was not obtained before first day of employment, from a state staff #3 has lived in within the past 5 years.
Plan of Correction: It was recently sent in, we will follow up if not received within 45 days. Results will be forwarded to our inspector.

Based on review of 5 staff records, the provider did not have a central registry finding within 30 days of employment.

A central registry finding was not obtained for staff # 1 & 2, within 30 days of employment, 11/21 and 1/22 respectively. Follow up on missing central registry did not occur until 1/22.
Plan of Correction: I followed up and waiting results. I will forward results to my inspector when received.
