Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 9/29/2025. The on-site inspection began at 10:55am and ended at 12:40pm. There were 91 children and 18 staff present during the inspection. The inspector reviewed 5 child records and 3 staff records on-site. This inspection included document review, interviews, and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the
violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 10/14/2025 . A POC submitted after this date will not appear on the public website.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following: work phone number and place of employment of each parent who has custody.

The record for Child #1, enrolled for 1 month, and Child #2, enrolled for 6 months, did not contain complete employment information for each parent.
Plan of Correction: The missing employment information for Child #1 and #2 was obtained and sent to the licensing inspector after the inspection.
