COMPLAINT RELATED
During the designated rest period, when children are resting or in an inactive state, the ratio of 1 staff per 20 children is permitted for preschool age children.

The Name to Face document for the PreK classroom showed on 1/20/2026, when PreK and Junior K were combined, a ratio of 1 staff with 27 children from 1:00-3:00pm.

On 2/12/2026, the inspector observed the PreK classroom during the designated rest period, when PreK and Junior K were combined, at 2:14pm. The inspector observed 2 staff with 28 children. Only 8 out of 28 were sleeping. The remaining children were observed sitting or standing on their cots, at the table, in the restroom, and walking around the classroom; not in an inactive state. Children were repeatedly reminded to stay on their cots. The third teacher signed out for break at 1:46pm and was due to return around 2:45pm.
Plan of Correction: On 1/20/2026 a teacher did not sign into the name-to-face sheet. Additional documentation of staffing assignments has been provided to the Department.

On 2/12/2026 ratio was corrected on-site - a 3rd teacher was placed in the classroom. Further discussion with supervisor clarified the meaning of "active" and "inactive". We will monitor when half the children are sitting up vs laying down on their beds.
