Inspector Notes

A monitoring inspection was conducted on 10/2/23 from 10:45 am to 11:55am. There were 6 children (19 points) present, with 2 caregivers. The children were observed playing with age appropriate toys, diapering and preparing for lunch. A total of 3 children's records, 2 caregiver's records, 1 household member's records and other family day home documents were reviewed. There were no children on site with medications and diagnosed allergies.

Standard 22.1-289.035-A
Based on records review, the family day home did not obtain results of a fingerprint based criminal record check (CRC) for employees, every five years.
Evidence:
1. The CRC for caregiver # 1 and caregiver # 2 were completed on 5/14/18 and 5/15/18, respectively.
2. Subsequent CRC searches were not completed in 5 years.
Plan of Correction: The fingerprints will be completed and a follow up will be conducted with licensing once the results have been obtained.
Standard 22.1-289.036-A
Based on records review, the family day home did not obtain results of a fingerprint based criminal record check (CRC) for adult household members, every five years.
Evidence:
1. The CRC for household member # 1 was completed on 6/5/18.
2. A subsequent CRC search was not completed in 5 years.
Plan of Correction: The fingerprints will be completed and a follow up will be conducted with licensing once the results have been obtained.

Based on records reviewed, the provider did not have a current certification in cardiopulmonary resuscitation (CPR).
Evidence: The provider's most recent CPR training was completed on 9/30/21 and was only valid for two years.
