Inspector Notes
A monitoring inspection was conducted on 10/2/23 from 10:45 am to 11:55am. There were 6 children (19 points) present, with 2 caregivers. The children were observed playing with age appropriate toys, diapering and preparing for lunch. A total of 3 children's records, 2 caregiver's records, 1 household member's records and other family day home documents were reviewed. There were no children on site with medications and diagnosed allergies.

Violations

1. The CRC for caregiver # 1 and caregiver # 2 were completed on 5/14/18 and 5/15/18, respectively.
2. Subsequent CRC searches were not completed in 5 years.

Evidence: The provider's most recent CPR training was completed on 9/30/21 and was only valid for two years.
