Inspector Notes

Conducted the annual religious exempt renewal inspection at 3:19pm. The inspection today was focused on the afterschool program. Observed 57 children with 15 direct-care staff. Ratios and supervision were in compliance. Children were observed arriving and departing from the program, washing hands, eating snack, and having outdoor active play during the inspection. A daily routine and schedule were evident. The program areas inside and outside were found to be clean and well maintained. Supplies and equipment were plentiful. Staff were actively engaged with the children and activities in all groups. An area of non-compliance was found with standards and laws reviewed. Based on records reviewed, 1 of 4 staff records reviewed did not include a required background check. Evidence: The record for Staff #14 did not include an out-of-state central registry search. There was no documentation that the search was initiated.
Plan of Correction: Staff member #14 lived outside of VA, requiring us to also reach out to OHIO for Central Release Registry. That process has already begun. We will follow up once results are in.
