Inspector Notes
An unannounced, on-site code compliance inspection was initiated on 04/02/2025 and completed on 04/04/2025. The on-site inspection began at 9:30am and ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were nine children present with two staff. The inspector reviewed five children?s records and two staff records on-site. Additional information was reviewed electronically on 04/04/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

1The national fingerprint background check for Staff #1 was obtained more than two months after their first day of employment.
