Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 4/04/2022. There were 8 children present and 2 caregivers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children?s records and 2 caregivers/household record were reviewed.

Standard 8VAC20-800-100-A
Based on review of two children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #2 (start date 8/16/2021) did not have a physical on file.
Plan of Correction: I will ask the parent for the physical.

Based on review of children's records, the provider had a child's immunization record, but the report did not include the signature by a licensed physician, the physician's designee, or an official of a local health department.
Evidence:
1. Child #2 (start date 8/16/2021) had an immunization record that was not signed and dated by a licensed physician, the physician's designee, or an official of a local health department.
Plan of Correction: I will ask the parent for the signed immunization record.
