Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 07/26/2024 and completed on 07/26/24, as a part of this licensure period. The on-site inspection began at 1:12pm and ended at 2:28pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 19. The inspector reviewed 9 children?s records and 2 caregiver records on-site. This inspection included document review and a tour of the family day home.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/02/204. A POC submitted after this date will not appear on the public website.
Violations

1. The provider is licensed for a capacity of only 9 children.
2. There were 11 children present.
Evidence:
1. Child #1 (3 years old) was napping on a FISHER PRICE infant rocker.
2. Child #2 (3.5 years old) was napping on a FISHER PRICE infant rocker.
Evidence:
Four children were sleeping on mats with no bottom cover.
Evidence:
1. FISHER-PRICE infant rockers are intended for use by infants and young toddlers with weight and age limits.
2. Child #1 (3 years old) was napping on a FISHER-PRICE infant rocker.
3. Child #2 (3.5 years old) was napping on a FISHER-PRICE infant rocker.
Evidence:
Child #3 (12 months old) had fallen asleep on an infant rocker and was not moved promptly to their crib.
Evidence:
1. There were 2 children present and the provider stated that they started today.
2. Child #10 did not have a record on file.
3. Child #11 did not have a record on file.
Evidence:
1.Child#4 (19 months old) was allowed to drink milk while lying down in their crib.
2. Child#4's milk bottle was found in their crib.
