Inspector Notes

A Subsidy Health and Safety inspection was conducted on 07/26/2024 to review supplemental health and safety requirements. The on-site inspection began at 1:12pm and ended at 2:28pm. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/05/2024. A POC submitted after this date will not appear on the public website.

Based on review and interview, the vendor did not maintain a written hard record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
Child #2, Child #3, Child #4, Child #10, and Child #11 were in care at the family day home and were not listed on the attendance sheet with arrival times for today.
