Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.
A subsidy inspection was initiated on 08/06/2021 and concluded on 08/27/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-580-E-1
Based upon review of five staff records and staff interview, the facility has not ensured that all staff have current certification in cardiopulmonary resuscitation (CPR).
Evidence:
1. The CPR certification for staff 2 expired on 11/30/2020.
2. Staff 6 confirmed that the CPR certification for staff 2 has not yet been renewed.
Plan of Correction: The facility responded with the following:
Staff 2 has been scheduled for CPR re-certification. A tracking sheet will be developed to ensure that staff renew CPR certification in a timely fashion.
