Inspector Notes

A monitoring inspection was conducted on 07/05/2022. There were 24 children present, ranging in ages from infants to school age, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the consultant during the exit interview

Based upon observation, the facility has not ensured that the most recent license is posted in a place conspicuous to the public.
Evidence:
The posted license in the entry foyer to the facility was the license that expired 02/17/2022. The Provisional License expiring on 8/16/2022 was not posted.
