Inspector Notes

An unannounced monitoring inspection was conducted on 11/14/2023 from1:00pm to 2:00pm. At the time of the inspection there were 26 children in care with 4 staff members. Children were observed napping. Five children?s and five staff member records were reviewed. Emergency drills, emergency supplies, required postings, background checks and staff qualifications and trainings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on a review of records and interviews, it was determined that the center did not ensure that employees shall undergo a background check prior to employment and every five years thereafter.
Evidence:
1. The records for staff #4 and staff #5 did not contain the required five year updated criminal history background check. The previous background check was dated 10/18/2018.
2.The Director confirmed the background checks were not in the records.
