Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/9/24. The on-site inspection began at 9:30am and ended at 11:15am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 24 children present and 5 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-350-B-4
The required ratio for children ages 3 years to school age is one staff per 10 children. Staff #4 was observed supervising eleven children (youngest child 3 years) from approximately 9:30am-9:55am.
Plan of Correction: Staff will notify Director when the ratio requires another staff member.
Standard 8VAC20-780-70
Staff records shall contain documentation to demonstrate that the individual possesses the education required by the job position. The record for staff #1 did not contain verification of education.
Plan of Correction: The staff member will bring in a copy of the diploma.
