Inspector Notes

An unannounced on-site monitoring inspection was conducted on 7/7/2022. There were 143 children, ages3 months--12 years, and 22 staff members and the director present during the inspection. The inspector observed the following: morning arrival, snack, free choice activities, diaper changing, and water play. A total of 5 children?s records and 5 staff records, were reviewed. The inspector arrived at 9 AM and departed at 11:15 AM.

Standard 8VAC20-780-210-A-1
Based on review of five staff?s records, the center failed to ensure that all program leads had the required education documented per the standard.

Evidence:
1. Staff # 1, 3, and 4 were working as a program lead and did not have documentation of High Diploma or equivalent.
Plan of Correction: The center director will obtain the missing documentation for the staff?s records and will ensure all records are up to date.

Based on review of five children?s records the center failed to ensure that all records were complete per the standard.

Evidence:
1. Child # 1 did not have, chronic physical problems completed on the enrollment form by the parent or the guardian.
2. Child # 3 was missing father?s information, chronic physical problems and any noted allergies.
Plan of Correction: The center director will obtain the missing information ASAP.
