Inspector Notes

An unannounced on-site self-report inspection was conducted on 08/1/2022. The self-report was received on 7/20/2022. The inspector interviewed two staff that was involved in the incident that was reported. The inspectors arrived at 10:50 AM and departed at 11:30 Am. There was not a preponderance of evidence to support the self-report of forbidden action. No violations were issued during the inspection.
