Inspector Notes

An unannounced on-site renewal inspection was conducted on 10/18/2023. There were 91 children, ages 4 months-4 years, and 18 staff members present during the inspection. The inspector observed the following: nap, and infant feeding and diaper changes. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: medication and program leader qualifications. The inspector arrived at 12:45 PM and departed at 2:30 PM.

Based on review of five children?s records, the center failed to ensure that all children had the immunization record prior to beginning attendance.

Evidence:

The record for Child # 3, with a documented first day of attendance of 09/25/2023, did not have the immunization prior to attending.
Plan of Correction: The parent will obtain a copy of the immunization records.
Standard 8VAC20-780-60-A
Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all the elements as required by the standards.

Evidence:

1. The record for Child # 4 has a diagnosed food allergy and did not have a physician completed food allergy action plan.
Plan of Correction: The center will have parent to obtain an allergy action plan.
