Inspector Notes
An unannounced monitoring inspection was initiated and completed on 06/02/2025, in response to a self-report received by the licensing office on 05/15/2025 relating to physical plant. The inspector reviewed 1 children?s record and 3 staff records on-site. This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1On 05/14/2025, Child 1 (age 10) was allowed access to a storage area where harmful substances were accessible to the child.
