Inspector Notes
An unannounced, on-site monitoring inspection was completed on 05/28/2025, as a part of the licensure period. The on-site inspection began at 11:20 am and ended at 12:25 pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 31. The inspector reviewed 4 children?s records and 4 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/04/2025. A POC submitted after this date will not appear on the public website.
Violations
5The sworn statement for Caregiver #3, who is currently working, was not signed or dated.
Child #3 and #4, who had been attending for 57 days, did not have documentation of their physical examination in their record.
Staff #3, who had been working for 57 days, only had record of one reference check.
Child #3 and #4 did not have the policy number included in their record. Both children have been attending for more than 2 months.
An infant under the age of 12 months was left unattended on a changing table during diapering for a period of 10 seconds, while the caregiver grabbed extra clothes for them.
