Inspector Notes

An unannounced, on-site monitoring inspection was initiated September 16, 2024 and
completed on September 16, 2024 as a part of the licensure period. The on-site inspection began at 8:44 am and ended at 11:19am. The inspector reviewed compliance in the areas listed above. There were 31 children present and 7 staff. The inspector reviewed 7 children's records
and 5 staff records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 26, 2024. A POC submitted after this date will not appear on the public website

Area and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.

The handlebars of two the scooter bikes were missing on the playground area accessible to children during outdoor activity.
A toy truck had multiple cracked areas with sharp points located on the playground accessible to the children in care during outdoor activity.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A container of disinfectant and dish washing liquid with the warning labels, "keep out of reach of children," were unlocked on the shelf in the kitchen area, the kitchen doors were unlocked and accessible to children in care.
Plan of Correction: Person-in-charge immediately 909-16-24) placed cleaning materials in a locked cabinet and will conduct walk-through checks more frequently to prevent reoccurrence.

A nonabsorbent surface for diapering or changing shall be used.

The diapering surface in the infant room was observed torn in multiple areas.
Plan of Correction: Person-in-charge immediately (09/16/24) replaced and changing pad during inspection and will monitor condition more frequently to prevent reoccurrence.
