Each staff member and individual from an independent contractor shall submit documentation of a negative follow-up tuberculosis screening at least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department. The record of staff #6 contained documentation of a tuberculosis screening that expired nine months ago. A follow-up tuberculosis screening had not been completed.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. On the school age playground, the resilient surfacing measured approximately four inches in multiple spots around the slide. On the preschool playground, the resilient surfacing around the dinosaur rocking equipment measured approximately three inches when at least six inches is required.

Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks and after toileting and any contact with blood, feces, or urine. Two children were observed going to the table to eat snack after being diapered. Neither child?s hands were washed after being diapered or before eating snack.
