Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 15, 2025, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were eight children present and two caregivers; the point total was 22. The inspector reviewed six children?s records and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

The record for Household Member #1 did not have documentation of a current Report of Tuberculosis Screening form. Documentation of the last Report of Tuberculosis was dated 09/11/2023 and was expired by one month.
Plan of Correction: Household Member #1 will make an appointment to complete a current Report of Tuberculosis form.
Standard 8VAC20-800-60-B
Children's records are required to contain the policy number of the child's medical insurance.

The record for Child #6 did not contain the policy number of the medical insurance.
Plan of Correction: The parent of Child #6 will provide the policy number of the medical insurance and document the record.
Standard 8VAC20-800-90-C
The family home shall obtain documentation of additional immunizations for a child not exempt from the immunization requirements and once every six months under the age of two years old.

The record for Child #1, 13 months, did not contain additional immunizations once every six months. Documentation of the last immunization was eight months ago.
Plan of Correction: The parent of Child #1 will be contacted to obtain a copy of the updated immunization records.
