Inspector Notes
An unannounced monitoring inspection was conducted on May 7, 2024, from 8:15am to approximately 9:35am. There were 6 children present totaling 14 points and three providers. The Inspector reviewed five children?s records and four staff records. Children were observed completing their Mother?s Day Art with paint and markers. No medication is administered and there are no allergies at this time of children in the center. There is no transportation given. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the provider at the exit interview.
Violations
2caregivers shall obtain a minimum of sixteen clock hours of training annually in
areas relevant to their job responsibilities.
Evidence:
1. In reviewing staff records, there was no documentation that staff obtained sixteen hours of annual training.
2. Staff #1, confirmed that all staff members are not up to date with annual training and lack hours.

Evidence:
1. Child #1 (date of Enrollment April 2024) did not have an up-to-date record on file at the family day home.
2. Staff #1 confirmed that Child #1 does not have an up-to-date record.
