Based on review of records, the center did not ensure the staff receive all required components of orientation training.
Evidence:
Staff 4's orientation training did not include the following topics: SIDS and use of safe sleep practices, prevention of shaken baby syndrome, traumatic head injury and food allergies.
Plan of Correction: This staff will receive all missing components of the training needed.

Evidence
One topical ointment for Child #5 expired in 4/2024.
Plan of Correction: If parent wants the child to continue the topical ointment, a new authorization will be obtained.
