Inspector Notes
An unannounced renewal inspection was conducted on 05/16/2023 from 2:10 p.m. to 3:45 p.m. At the time of entrance, 9 children (19 points), ages five months through five years, were in the care of one caregiver. An additional caregiver arrived at 2:27 p.m. and a third caregiver arrived at 3:40 p.m. Three caregiver records and five children's records were reviewed, as well as one pet record. No medication is being administered at this time. All interactions between children and staff were age appropriate and positive. Some items that were reviewed and observed included: required posted information, documentation of evacuation, shelter-in-place, and lockdown drills, emergency supplies, first aid kit, warm water temperature, updated emergency preparedness plan, hand washing and bathroom procedures, and transitions.
An exit meeting was held during which the preliminary findings were reviewed and the provider was given the opportunity to present any items that were not located during the inspection and ask questions. Areas of non-compliance are identified on the violation notice, including repeat violations.

Violations
2Evidence:
1. Upon the arrival of the Licensing inspector at 2:10 pm on 05/16/2023, there were nine children in the care of Staff #2, totaling 19 points.
2. Staff #2 confirmed that she was the only caregiver to the nine children (19 points).
Based on record review, the provider did not ensure that a child's record contained all of the required information.
Evidence:
1. The record for Child #1 was missing the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
2. The record for Child #2 was missing the first day of attendance and the name, address and telephone number of one designated person to contact in case of an emergency if the parent cannot be reached.
