Inspector Notes

An unannounced, on-site renewal inspection was initiated and concluded on June 5, 2025. The on-site inspection began at 10:30 a.m. and ended at 11:35 a.m. The inspector reviewed compliance in the areas listed above. The caregiver was not present and the children were in care of the assistant caregiver. There were 7 children present and 1 caregiver; the point total was 11. The inspector reviewed 5 children?s records, and 2 caregivers records on-site. This inspection included document review, tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 16, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. There is a plastic play truck outside that had visible cracks that could snag skin. The plastic play house contained cracks and had visible dirt and debris in the sink portion and the table. There was a sectioned off portion of the backyard with several broken pieces of lattice that were sharp and splintered. There were rusted, protruding nails on the lattice and decking of the home. The deck has chipped paint.
Plan of Correction: The plastic play truck that had a crack has been removed. The playhouse will be removed. Asap the lattice will be replaced. And deck will be stain asap.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Disinfectant wipes were on a low shelf on the diaper table accessible to children.
Plan of Correction: The disinfectant wipes were placed back on top shelf.
Standard 8VAC20-800-320-H
All flammable materials shall be stored in an area inaccessible to children. Two propane tanks were in the yard where children were playing and had access.
Plan of Correction: Propane tanks were removed and place in storage.
Standard 8VAC20-800-500-D
Repeat Violation
Outdoor play equipment shall meet the following requirements: "S" hooks on swings shall be tightly closed. The "S" hooks on a swing were not tightly closed and had an opening that allowed a finger to fit.
Plan of Correction: The S Hooks are closed on the swing.

The following telephone numbers shall be posted in a visible area close to the telephone:

Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. There was no documentation of a fire drill in December 2024.
Plan of Correction: The emergency evacuation procedure sheet along with the fire drill sheet has been turned in and are practice monthly.
