Inspector Notes

A monitoring inspection was initiated on 7/29/2023 at approximately 9:50am and concluded on 7/27/2023 at approximately 10:45am. There were a total of 9 children (27 points) observed with 2 caregivers present. The children were engaged in the following: circle time, singing, toy play, games, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 9 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, pressure mounted gates were used as protective barriers at stair openings.

Based on review of children's records, telephone numbers of two designated persons to contact in case of emergency were not kept in children's records.

Evidence: Telephone numbers of two designated persons to contact in case of emergency were not recorded for child #9.
Plan of Correction: Provider will obtain telephone numbers of two designated persons to contact in case of emergency for the child.

Based on review of children's records, immunization records for a child were not obtained every six months for children under age of two years.
