Inspector Notes

This inspection was conducted by Licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 5/10/2021. The Provider was contacted by telephone to initiate the inspection. There were 10 children present, totaling 24 points, with 2 caregiver staff. The Inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 caregiver/household member records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and one violation was documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record C does not include documentation to show that the parent has reviewed the child's contact information within the past year. This information was most recently reviewed by the parent on 1/1/2020.
Plan of Correction: I will ask the parent to review the form today at pick up.
