Inspector Notes

An unannounced monitoring inspection was conducted today between 12:30pm and 1:50pm. There were 12 children present with the Provider and 2 assistants. The children were observed napping. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during todays inspection and can be viewed on the Violation Notice.

Standard 22.1-289.058
Based on interview, the family day home is not equipped with at least on carbon monoxide detector.
Evidence: When asked, the provider stated that they did not have a carbon monoxide detector installed.
Plan of Correction: We don't have any gas in the house so I didn't think we needed it. I have one but it isn't installed. I'll install it.
Standard 8VAC20-800-480-D
Based on observation, equipment used by children were not used in accordance with the manufacturer's instructions.
Evidence: An infant was observed in the sleeping room, asleep in a mobile, rocking bassinet. The bassinet included a padded head rest and the head rest was covered with a blanket. The manufacturers warning on the side of the bassinet states that blankets should not be used in the bassinet and that children should never be left unattended while sleeping in the bassinet.
Plan of Correction: This baby does not sleep in a crib at home and needs the motion of the bassinet to sleep. I will talk to the parents about starting to get [them] to sleep in a crib.

Based on record review and interview, children's records did not contain all required information.
Evidence: Child #5's record does not contain the families medical insurance policy number.
Plan of Correction: I will ask the parents to provide this information.

Based on record review, the family day homes does not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
1. The USDA recommends that children are offered a serving of a fruit or vegetable for breakfast. The providers food menu does not offer a fruit or veggie at breakfast time.
2. The USDA recommends that children are offered a serving of fruit and a serving of vegetables for lunch. The providers food menu does not offer a fruit with lunch on Monday-Friday for the current week, and does not offer a vegetable with lunch on Tuesday or Friday for the current week.
