Inspector Notes

An unannounced monitoring inspection was conducted February 13, 2023 between 12:15pm and 1:45pm. There were 8 children present with 2 caregivers supervising within the supervision guidelines. The children were observed eating, preparing for nap and sleeping. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, staffing and supervision, programing, medication, fire drill log, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs. Based on record review and interview, annually, the provider did not review with the parent the emergency contact information required in 8VAC20-800-60-B2 to ensure the information is correct and obtain the parent's signed acknowledgement of the review.
Evidence: The last review was documented on 6/10/21 on the emergency contact information for Child #1.

Plan of Correction: I will speak to the parents and get them to review and sign the acknowledgment.
Standard 8VAC20-800-90-C
Based on record review, the family day home did not obtain documentation of additional immunizations for a child once every six months for children under the age of two years.
Evidence:
1) Child #1 (DOB 12/12/20) record did not have additional immunizations every six months. Last update was 12/31/21.
Plan of Correction: I will speak to the parents today and tell them to bring in the updated immunization by Monday.
